The problem was that the original design had long bolts supporting BOTH walkways (one walkway was directly above the other). A single long bolt would have had BOTH walkways bolted to it. Each walkway would only need to support it’s own weight.

The installation process would have required installing the topmost walkway through the long bolts, then installing the bottom walkway through the long bolts.

The contractor did not believe that they would be able to do that without damaging the threads on the long bolts to the point where they wouldn’t be able to install the lower walkway.

A design change was made and the new design had the top walkway supported by shorter bolts that did not continue below the top walkway. Additional bolts were added to the top walkway to support the bottom walkway.

Now the top walkway had to support the bottom walkway, which it was NEVER designed to do....and so it collapsed.

You’re both off a bit on that story.

The walkways were designed to be suspended from the ceiling by a series of continuous hanger rods that ran through all of them and transferred all the weight directly to the ceiling. The company that was supposed to fabricate them objected to the original design because it would have required the rods to be threaded for their entire length in order to screw on the nuts that would hold each walkway deck in place.

The engineer on the project allowed them to revise the design so that each walkway floor was suspended from the one above it by a separate set of rods bolted in place to the floor deck above it. This resulted in an overload on the top walkway deck because it was being forced to hold up its own weight plus the weight of all the walkways below it.

The Wikipedia article on the subject has a good illustration showing how the design change altered the load on the bolted connections and the rods.
