There is a real Dr by that name.
I used to work there, for a week as a contractor. I worked in one of their northern offices.
I’ve seen their EMR (Electronic Medical Records (system)) and it didn’t look like this. Now it was about a decade ago, so they likely invested in a new EMR since I was there.
There are two things I would like to talk about and share with the group:
1. Letterhead should be locked up in Medical and Legal practices. Letterhead is supposed to be as good as a signature - a symbol that what is written on the paper is the official action of that office. NOT ONE OFFICE I have ever seen follows this rule. The letterhead is just sitting around for anyone to take.
2. A doctor reports directly from the EMR. So you call up a patient record, go to File-> Print and it prints it out. Doctors don’t go into Word and print things like this.
To sum up, imagine your electric bill. Your electric bill has sections. Boxes with your account info, contact info, paragraphs and formatting. It looks like a bill. Your power company will never print out a letter that says “Mr soandso has paid the bill for April. Your next bill is due in May for the amount of $400. Thank you and have a great day.
Love,
Electricity.”
This letter isn’t printed from any EMR that I know of. Now I don’t know every EMR in the world, and not every screen in every EMR in the world but a print out in this format from an EMR is highly unusual.
This looks more like someone copy-pasted, grabbed company letter head and printed it out in Word. And that IS technically a forgery. It’s not authentic at all.
A Doctor would never print out something like this. A doctor would never print out a sheet of paper that said “Hillary Clinton is retarded. Love, The Doctor.”
This form is a forgery, even if it came from an ex employee and contains true information.

Exactly... me too.. I’ll be the last one on paper... but if she is head of a department as indicated on the letterhead it’s likely a big organization and they would require EMR for maximum reimbursement and “meaningful use” cash.
All the other doctors carry a laptop around with them. He carries paper files. I’m sure he has someone who does computer input for him at the end of the day.
Well, I guess that person’s 40 years beats my 39 years!
