I'm not so sure about the head, but the scarf makes the neck disappear and produces a strange effect.

Regarding the head: Obama *is* a naturally scrawny guy. I would imagine that in his youth he was really thin, which would make his head look big. Now that he’s 50, he should have picked up a little weight and “filled out”.
