Very possible.
Here is my more innocent theory — it is indeed a fully functional firearm. Someone or someones took it out to the range to test it out. Maybe because it was cool. It looked cool.
After shooting it someone left a round in it. Stupid them. This can easily happen with an auto where a round is still chambered even after the mag has been removed.
Not clear if it was a revolver. If it were, then a similar situation might have happened. Same point.
Here is my expectation:
If a gun is used on set, it either has to be disarmed completely or not operational in the first place. And whomever supplies the firearm has to warranty it as such.

I think I first saw it as a story line for a Columbo episode about 45 years ago. People must really hate that guy.
