Too many people die from criminal killing them—I say use a gun—face instant death by Cop-Death Squads. Maybe they could make it look like an accident? Put them in a car with a stuck excellorator and have it hit a tree. Maybe put soming in the car so it will burn up in a big fireball and cover up the evidence of a crime? Yeah, maybe have Union Thugs or Moslem Brotherhood members do it!

It comes out of the current quest for safety in the petrochem world. Basically no matter how many systems or how much training an organization has it will reduce the severity of accidents but sooner or later there will be a fatality that will fall outside of the “preventable” model.

There is a lot of debate about it but there have been examples of cases where a severe injury or fatality happened with no witnesses and/or no equipment failure occurred. Essentially an admission that $hit happens even when you try to manage it out of the system.
