Mickey: Cirrhosis of the liver with jaundice! Alright I get to wear make-up! What did you get?

Kramer: Gonorrhea? Excuse me, I think there’s been a mistake, see, I had gonorrhea last week.

Dr. Wexler: Oh, it’s no mistake. We loved what you did with it.

Kramer: I don’t believe this, I’m being typecast.

They should have a safe word (like Palomino) to let the others know that it is for real and not just an act.

Good idea. But you'd need a different safe word for "Equus".

I believe that this did actually happen to actor/commedian Dick Shawn, who died while on stage during a standup comedy act.

From Wikipedia:

Foxx appeared to be making a comeback with the 1991 series The Royal Family, in which he co-starred with his long-time friend Della Reese. During a break from rehearsals on October 11, 1991, a fatal heart attack felled him on the set. Reportedly, Reese and the rest of the cast and crew thought he was doing his classic (and in this case, ironic) “I’m coming, Elizabeth” fake heart attack routine he made famous on Sanford and Son, even going as far as collapsing to the floor, although that was not part of the usual schtick.[3] However, this heart attack was real, and Foxx never regained consciousness. He was 68 years old.

I think the best safe words would be “Holy Cr@p! I’m having a heart attack!”

Okay, you can get up now, Steve.
Steve...? Cut the clowning, Steve.... STEVE.

(Sheesh, this could be a great story line for a play - - actor acts like he is having a real real heart attack while his character is acting that he is having a heart attack. Nevermind - - my head is already spinning.)

Did this ever happen to Redd Foxx?

Actually yes, it did.
