1. Deregulate the state exchanges, while capping subsidies.
2. Slowly shift Medicare patients into the exchanges.
3. Let more people buy insurance on their own rather than through their employer.
4. Move Medicaid patients into the exchanges.
The result would be a more coherent system where more Americans would be buying health insurance on their own from private insurers competing hard for their business.

Then again, anyone who thinks government run exchanges with politicians and bureaucrats making the rules is “free market” needs remedial studies

To some degree, that is already done with Medicare Plan D for medications. For the first time, I had to participate in this to get help on an expensive medication, and found out that the participant can select the insurance provider, as well as the pharmacy, local or mail-in. This has put pharmaceuticals and insurances on a competitive basis for the Plan D portion. The insurance cost is dependent on the particular medication costs, and can be taken out of the Social Security check.
And medication is the first line of defense in preventative medicine. Always thought George Jr. Was a genius for cutting medicare costs with an affordable drug program of choice.
Many states are not opening exchanges. The states that are are mostly leftist and are not going to do anys such thing as those suggested.
