The modern day Clara stepped into the rip in space-time that was left after the Doctor’s final death, trying to save him from the “Great Intelligence” creature that stepped in and started killing him at all points in his life.
She then was recreated over and over again through time, creating multiple Claras that we’ve seen over the show.

And in the movie "World War Z", he played a W.H.O. doctor - ironic? I saw that. Very ironic!
