Actually, by now I think we're in the double dog dare phase.
I'm not talking about their citizens in general. I'm talking about the fact that nuclear facilities need engineers, scientists, and trained technicians to keep them working. If the facility survives but the personnel don't, it does them no good.
Hell, you don't even need to blow up the facility; just blow up the parking lot at shift change.
Much as I dislike the mullahs, they have not managed to destroy everything Iran built up under the Shah, so while life is atrocious presently under the current regime, they still have some fairly sharp, perhaps misguided, minds available.
Even if they can only "monkey do as monkey see", they have acquired nuclear information and technology from Pakistan, North Korea, former Soviet states etc., to an extent where it's more a matter of implementing things - not inventing anything.
And you are so right about the sewers. The cities or villages rest on hundreds of thousands of septic wells - without any chemicals in them to help kill germs and bacteria which then seep into the surrounding earth and pollute ground water.
He's bluffing. Their facilities can all be destroyed or disabled with EMP bombs or by repetitive bombing in the exact same locations with GPS-guided bunker busters. The bunker busters would work like a mining operation--each bomb makes the crater another 10-20 feet deeper until we finally reach the underground nuclear weapons facility. GPS guidance has made that kind of repetitive bombing much more effective. EMP bombs would leave structures intact but destroy all electronic equipment inside.

Bingo. Probably the same people that keep trying to get the US to hit Iran militarily.
