This is probably nothing.
If its photo manipulation, its certainly an incredibly mundane example, and I cant think of a reason why anyone would do it.
Im tossing this to the lizardoid community for comment from the other photoshop geeks out there, without rendering any judgment yet.
In other words, I am not saying this is a definite fake; I know weve got quite a few experts at digital manipulation among our readers, and Im sincerely asking for their opinions.
But look at the image of the man in the lower right corner; the more I examine it, the more it resembles a mans head crudely pasted onto the body of a woman. Note the scarf under his chin, the outlines of something else at the right of his head, and more artifacts at the top and left of his head.
And note the rest of the clothing, which resembles the other womens clothing much more than the mens. The lighting of his face seems off, as well.

What's going on here is an attempt to elicit sympathy from African-Americans ~ it's like, well, all these A-rab pictures look like white folks!

Although the Palestinian propagandist (who posted this picture on the net) hoped to lead us to be concerned about the little kid, it's pretty clear the NYT picture-guy saw it entirely differently. Little did the propagandist know that NYT people have hard hearts when it comes to kids (the abortion-thing doncha' know ~ can't get all smurfy about kids either), and are much more concerned with PC stuff, like "proper count of racial types" in their photos.
Sometimes I have to pity these funny little foreign people as they attempt to dealve into the American Liberal psyche. It's not at all the same as the Islamofascist terrorists with whom they normally work ~ it's just wierd!
They needed a "racial diversity individual" (as in token black) to please U.S. libs.
There is writing on the man's shirt. Could that explain why he's photoshopped in? A message to somebody?

Ok, I checked out the picture at Little Green Footballs, and here's my best guess.
The original picture was a doctor, maybe a US doctor, who didn't want his picture in the paper. The place was a hospital, and it looks as though that could be hospital scrubs over the left shoulder and a brown, collared shirt over the right shoulder, and possibly a hospital type mask pulled down around the neck. I have no idea why the poster didn't put the brown shirt over both shoulders.

Sorry, I thought this was the claim that the smoke in the city not being all that big a deal.

I'm not an expert, but being a novice PS dabbler does make me an expert on what looks crappy. That head doesn't go in that picture. The white dot on the top of his head matches the white scarf under his neck. The stuff on the right of his head is just a bad lasso tool application. Besides that, his head is twice as big as the people standing next to him.
Yes, I agree. This looks fake. The stuff on the right of his head (the viewer's right) looks like it was background retained from the photo from which the head was clipped.
Something doesn't look right with that version. The guy in the back, his eyebrows are too close together. I'll study it a bit more. :-)

The photoshop was to cover up a woman who didn't have her head properly covered.
