I’ll side with Robbins and Sarandon here. As someone who lives a couple of blocks from an area where a huge hospital is growing like a cancer, obliterating neighborhoods of beautiful old homes that it regards as annoying obstacles to its never-ending expansion, I absolutely oppose this trend. We need more mega-hospitals like a hole in the head, and the people who are heading up the mega-hospital projects are most certainly not doing it out of a desire to “provide care to the indigent”. And no, I’m not impressed with orchestrated demonstrations by unionized workers — they aren’t in this to “provide care to the indigent” either.
Make no mistake about it, there is one thing and only one thing driving the trend to ever-larger hospitals, and that’s socialized medicine. Patients are processed by the hundreds like commodities, treated by overworked young resident doctors who are paid slave wages and ordered to work in whatever location the national match system sends them to. Private companies spring up to sell endless tests and procedures inside and on the immediate perimeter of the hospital, not according to what patients are most in need of, but according to what tests and procedures can be churned out at the highest reimbursement to cost ratio, from Medicare/Medicaid and from intensely regulated “private” insurers. Little details like controlling the spread of infectious agents fall by the wayside as doctors and nurses and technicians race around these huge complexes making fleeting contact with hundreds of patients of day, few of whom have any choice whatsoever as to where or how they receive their medical care. No thanks. The faster this scam collapses, the better.

Sarandon has earned enough money that she should be able to shelter herself to whatever extent she wants. John Edwards is lucrative enough that his 25,000 sq.ft. compound is entirely fine. John Travolta is successful enough that he can own and fly a 747 at his leisure. God bless American free enterprise.
A problem only arises when people like Sarandon, Edwards or Travolta deign to lecture average Americans about the exact same behavior. Edwards gives a speech on “two Americas”, and lives in a gigantic estate. John Travolta lectures me about my Suburban’s “carbon footprint”, and he flies a 747 (shut up). Sarandon talks as if I should let the homeless bunk in my garage, and then balks at anything in her backyard.
I don’t begrudge success ... God bless capitalism ... I just ask for the same opportunity they demand for themselves.

What makes this funny is Sarandon and Robbins are the very first to lecture us on the joys of socialized medicine and providing government funded care to everybody in infinite quanitities. Then when the infrastructure to deliver that care is constructed, they are a bunch of NIMBYs.
I am not advocating hospital expansion when I point out that this is a contradiction.

What’s the benefit to the hospital. To build a mega hospital requires extensive borrowing. With socalized medicine the funds won’t be available to repay the loans. Where’s the gain for a hospital.

GS' criticisms of the mega-hospital trend are cogent, but St. Vincent's is not the stereotypical offender.
Dig a little deeper into the finances of our health care system. Things like this are always driven by specific parties who stand to make a bundle of money. The fact the hospital business itself is a guaranteed money loser is irrelevant.
For a good, but unfortunately outdated dissection of the problem, I recommend the book “The Hospital that Ate Chicago”. It’s a satirical yarn, but built on financial details that were absolute reality in the 1980s. The financial details have changed, but not the basic outline of the scam.
