MEXICO CITY Can a pill a day help prevent infection from H.I.V., the virus that causes AIDS?
No one knows. But researchers in a number of countries are conducting trials and planning others to test the unproven strategy that a daily pill, or a combination of drugs, can prevent H.I.V.
By mid-2009, more people will be enrolled in such trials than in all of those for H.I.V. vaccines and microbicides, the AIDS Vaccine Advocacy Coalition said in a report issued here on Sunday at the start of the 17th International AIDS Conference.

The United States Agency for International Development, the C.D.C. and the National Institutes of Health are paying in part for all of the trials. The Bill & Melinda Gates Foundation is paying for part of two. The organizations have undertaken the trials because of the success in giving antiretroviral drugs to pregnant women to prevent H.I.V. infection in their infants and drugs to prevent malaria.
Initial PrEP studies are testing tenofovir, either alone or combined with another drug, emtricitabine.
Infected people taking these licensed drugs have shown limited side effects like nausea, vomiting, diarrhea and intestinal gas. But the safety of the drugs must be established among noninfected people as well as among participants who become infected in the study.
Trial participants include gay and bisexual men, heterosexual men and women, couples in which one partner is H.I.V. infected, and sex workers.
Up to 15,000 people are expected to be participating in trials by mid-2009. Sites of the trials include Botswana, Brazil, Ecuador, Kenya, Malawi, Peru, South Africa, Tanzania, Thailand, Uganda and the United States.
Male circumcision has proved effective in a significant percentage of H.I.V.-negative people in trials in Africa...

It’s rather paradoxical, isn’t it? They’ve got a disease that supposedly wreaks havoc on the immune system, so they fight it by taking drugs that wreak havoc on the immune system.
There really are times ( like this ) when I feel like I've jumped aboard The Crazy Train, and would like to jump off.

For over two decades the Public Health Establishment has used your tax dollars and the full power of the federal government to wage a massive propaganda (and intimidation) campaign designed to (A) dupe the American public into believing that HIV is the cause of AIDS (B) scare the public into thinking "we are all at risk" (C) coverup the extreme toxicity of AIDS chemotherapy drugs (which are not just used on "fast-track" gays and junkies, but also given to pregnant mothers, infants, and children) (D) use this fear to push a leftist social agenda that includes socialized medicine, and the promotion of homosexuality and explicit sex "education" to tender-aged school children (E) use their "public health mandate" to bypass the authority of parents and local school boards who object to their social engineering schemes (F) create a massive federal bureaucracy encouraging the use of addictive drugs, to include prescription heroine (G) use threats and intimidation to silence dissenting scientists and to keep the press from covering the debate (H) and finally, to use AIDS as a model to push similar social agendas with respect to future epidemics.

No. Risking adverse drug reactions for an otherwise avoidable disease is just stupid.
I wouldnt hang my hat on some studies from Africa like Duesberg does. There are other well done studies from Africa that show circumcision cuts the rate of HIV/AIDS by half or more. Before HIV/AIDS, most IDU/IVDA died from drug overdoses, not rare opportunistic infections. You still havent explained the dramatic decreases in vertical transmission, mother to child, in HIV positive mothers with only perinatal antiretroviral drug treatment.
Hmmm. I think I'm going to talk to my oncologist about starting a program of aggressive chemotherapy and radiation treatments--just in case I get cancer some time down the line!
Vertical transmission of what? HIV or AIDS? I'd expect the former, but it would be interesting to see the stats on the latter.

The only people who are HIV positive that don't get AIDS have a genetic deletion for the chemokine receptor CCR5. "In epidemiological studies, the allelic frequency of the CCR-5 gene deletion was 10-20% among caucasians, particularly amongst those of Northern European descent with 1% homozygosity." The gene for the chemokine receptor CXCR4 has a number of mutations called single nucleotide polymorphisms that also effect the development of HIV/AIDS. These receptors are co-receptors for HIV. If you want to understand it, then read the following paper at the first link. I can't help it if it is heavy on immunology, genetics, etc.
Enter perinatal HAART and HIV vertical transmission into PubMed's query box, if you want more articles. The newest, i.e. first, articles dwell on adverse drug reactions. The many older ones show the effect of these drugs in preventing vertical transmission.
Duesberg can afford to be cavalier with his hypothesis. He's not a physician. Find me a physician who believes like Duesberg and still practices medicine. They will be doing it without a license.
That all sounds well and good. But to my knowledge, there has not been a single study conducted to determine how many HIV-positives stay healthy. Nor will such studies ever be allowed, so long as the AIDS establishment is in charge IMHO.
And even if there are such studies, they're conducted on people who are known to be HIV positive, which means they're probably being treated with antiretrovirals, with all that that implies.
You are absolutely correct...AIDS chemo drugs, or lack thereof, would definitiely need to be controlled for.

Why don't you look at mortality statistics before 1987 when AZT was first approved in this country. There was nothing before AZT. You can limit the years in which you are interested at PubMed.
Their T "helper" CD4 positive cells were selectively destroyed and they died unless they had unique genetics that prevent entry of that virus.

An NCI grantee scientist, Dr. Peter Duesberg of California/Berkeley, has published a paper in a scientific journal which concludes that the HTLV-III/HIV virus identified by Dr. Gallo and Dr. Montagnier is not the cause of AIDS and that the disease is caused by “a still unidentified agent” which may not even be a virus.
Inexplicably, the paper was published in the March 1 addition of Cancer Research, and gives a non-specific credit to Dr. Robert Gallo and others, but nobody within the Department or the news media seems to have been aware of it until it was disclosed Monday, 4/27, by a gay publication in New York City.
Dr. Duesberg has been an NCI grantee doing research in retroviruses and oncogenes for 17 years and is highly regarded. He is the recipient of an “outstanding researcher” award from the Department. The article apparently went through the normal pre-publication process and should have been flagged at NIH. Failing that, it should have caused a splash on publication nearly two months ago.
Playwright, gay activist and Department critic Larry Kramer is currently bringing it to the attention of the media, but it really hasn't taken off yet. I know for instance he has talked to Tom Brokaw about it. There has been one call to CDC from Newsday and none to the press office so far.
This obviously has the potential to raise a lot of controversy (if this isn't the virus, how do we know the blood supply is safe? How do we know anything about transmission? How could you all be so stupid and why should we ever believe you again?) and we need to be prepared to respond. I have already asked NIH public affairs to start digging into this.
