Last year, the publicity that followed Helen Mirren's portrayal of the monarch in an award-winning film and the sheer length of her time on the throne saw her move up from 46th place to 23rd.
But this year, she has been pushed 35 places down the list to 58 by an influx of new women in charge of the world's financial, oil, food and mining firms.
The Harry Potter author JK Rowling, Madonna and Margaret Thatcher did not feature at all in the list.
The rankings are based on a combination of influence, economic impact and media "reach".
The highest new entry is Sheila Bair, who is head of the US Federal Deposit Insurance Corp which steps in to prevent banks going under, and comes second only to the leader of Europe's biggest economy, German chancellor Angela Merkel.
Other high-ranking women include the chief executives of firms dealing in oil, food and commodities.
A total of five UK-based women are ranked among the world's most powerful this year although only four are British nationals.
The highest ranking, up two places at number five, is American Cynthia Carroll, 50, the chief executive of mining giant Anglo American and a mother of four.
She is followed by Dame Marjorie Scardino, the chief executive of publisher Pearson PLC, which owns Penguin Books and The Financial Times newspaper, at number 20. Scardino, 61, was born in Texas but became a British citizen.
London's Olympic Aquatics centre architect Zaha Hadid follows at number 69. The last Briton on the list is Clara Furse, the chief executive of the London Stock Exchange, at number 85.
Forbes editor Chana Schoenberger said the Queen's position was affected more by the women she shares global power with rather than anything detrimental she herself has done.

Elizabeth I was a powerful woman. Elizabeth II is a rich one, but not really very powerful. Madonna is just a whore who spent her most productive years running in front of the bandwagon ...until she gets run over by it.

These kinds of lists are always very subjective anyway. How do you define “powerful” anyway? Is there not power over peoples thoughts, imaginations, and beliefs? Is that more or less powerful than economic clout? How do you compare such things?
