As others have noted, he has done this before to Hillary. Twice as he brought up her name as he "congratulated" her and her effort. This one with McCain is a carbon copy except in this instance Obama goes first with the index finger and scratches and then switches to the middle finger. He either forgot to initially go with the middle finger or he used his index finger first as a cover.
The point, newbie, is the moment he did this was as he "congratulated" McCain just as when he was "congratulating" Hillary. It's supposed to be clever in a plausible denial kind of way but comes across loud and clear - underneath his supposed cool demeanor rages a person with no real soul but an angry socialist thug born of an unstable childhood and the most nefarious of mentors that molded him into a clandestine candidate.
