According to the official reference book pdf you are to mark one or more boxes regarding race.

And you should:

"Mark the “White” box if this person has origins in any of the original peoples of Europe, the Middle East, or North Africa. This includes people who indicate their race as “White” or report entries such as Irish, German, Italian, Lebanese, Near Easterner, Arab, or Polish."
"Mark the “Black, African Am., or Negro” box if this person has origins in any of the Black racial groups of Africa."

Obviously 0 has white blood, so he either can't read the directions or hates his white ancestry so much he can't bring himself to check the box. Since he is a complete screw-up I'm inclined to go with the former but I don't rule out the latter.

If you don't want insurance to pay your bills, don't use insurance. Pay the bills yourself and don't sign the form authorizing release of information to the insurance company.

If you don't want your wife to pick up your medical record, pick up your medical records yourself and don't sign the form authorizing her to do so.

If you don't want a second or opinion or a referal to a specialist, fine. Don't sign the permision forms for release of information to the specialist.
