I've always heard that it came out of the LBJ White House.. Bill Moyers in fact has been mentioned as the source.
I recall that the Republicans protested after the first nationwide sponsored showing and the Democrats dropped it --kind of. Actually they handed it over to network news who featured it free, it seemed to me, every night as a news item until election day.
IIRC, the ad only ran once, but was talked about frequently, and at least partially run on the “objective” networks.
In the words of Nelson Rockefeller's public relations head, Stuart Spencer, "We had to destroy Barry Goldwater as a member of the human race."
The feelings were mutual. I wish I could find that photo of Nelson at the podium giving the finger to Goldwater delegates interrupting his speech where he was trying to get a plank in the 1964 platform virtually accusing Goldwater of being a member of the KKK.
I have posted this several times including suggesting that the Party, et al. intend to do the same things to Gov. Palin.

I'll just add that it may have been the starting point of the harshly negative campaign ads but it followed the harshly negative news coverage such as CBS suggesting that Goldwater hurried off to Germany after the convention to meet with Neo-Nazis and visit a place where Hitler used as a home (the latter was true but after W.W.II the place was a U.S. military installation but CBS did not mention that. Schorr I think was the CBS employee doing the harshly negative news coverage.)

(Years ago) Jacques Nasser as chief executive officer of Ford Motor Co., ... told a company meeting of employees "I do not like the sea of white faces in the audience and FoMoCo must ensure that in the future the company reflects the broad spectrum of Fords customers."
Shortly after that Jacques Nasser was no longer CEO and FoMoCo remembered that there are a lot of white people in America.
Europeans will do no less.
