Come 2014, you'll have to pay a tax if you're uninsured -- but how exactly it's going to work is still getting sorted out. The IRS isn't sure how many people they'll have to hire, or how much implementation will end up costing:

It is really bad when you think about the people they will be going after. Just above the poverty line and lower middle class. They cannot afford a tax attorney. They have no idea what it will be like. And the IRS can make it very, very personal. They did exactly that to me last year.

Probably like how the State of Ohio verifies you have auto insurance. They send you a letter telling you you have X number of days to mail them a copy of your auto insurance card. Either that or you will have to attach a copy of your insurance card or a form from your insurance company to your 1040.
