Israel has revealed very little about the man known as Prisoner X, an Australian-Israeli who was jailed here in 2010 after apparently getting caught in a web of espionage and intrigue, other than how he died: alone, by asphyxiation, after hanging himself in the shower of his cell.

I guess this just goes to show that not only the CIA has the ability to occasionally recruit incompetents.
Probably all intelligence agencies, with the possible exception of the GRU, are incompetent. MI5 even had a likely Soviet mole in charge during the height of the cold war.
M15 was stacked to the rafters with double agents during the 40’s and 50’s.
Even the US precursor to the NSA had a Russian mole in the decoders office who discovered in 1948 that the US had broken the Russian code in the venona operation. The russians changed their code subsequently. So all the venona cables refer only to the period prior to 1948.
