Transgressions such as this are rarely revealed publicly. But officials with direct knowledge of Air Force intercontinental ballistic missile operations told the AP that such violations have happened, undetected, many more times than in the cases of the two launch crew commanders and two deputy commanders who were given administrative punishments this year.
The blast door violations are another sign of serious trouble in the handling of the nation's nuclear arsenal. The AP has discovered a series of problems within the ICBM force, including a failed safety inspection, the temporary sidelining of launch officers deemed unfit for duty and the abrupt firing last week of the two-star general in charge. ...
"The only way that you can have a crew member be in 'rest status' is if that blast door is shut and there is no possibility of anyone accessing the launch control center," said Lt. Gen. James Kowalski, the commander of Air Force Global Strike Command.
The written Air Force instruction on ICBM weapon safety, last updated in June 1996, says, "One crewmember at a time may sleep on duty, but both must be awake and capable of detecting an unauthorized act if ... the Launch Control Center blast door is open" or if someone other than the crew is present.

Almost feels like these sorts of things are being done intentionally so when a nuke goes off in a major city they can claim it was a nuke stole by some “teabagger/patriot type/far right extremist/liberal created boogieman”. Almost like they are creating a narritive ahead of time to excuse them and blame their political enemies in leu of the actual people who will probably end up doing it such as some turd world near-russia-stan country that would have smuggled it across our intentionally porous southern borders....
A clear violation of the Two Man Rule; There shall be two men at all times on watch to safeguard access to or work preformed on nuclear capable systems. In the launch control command center, there are two officers on watch at all times and both are armed. There are two relief officers. Each watch is 24 hours on and 48 hours off, 365 days a year.

There is a lot of security at the sites so someone already on the inside had to rat them out.
In my early Air Force career, I worked communications at Titan sites and you just can’t walk in to the level these two were at without going thru a few security checkpoints.
Good questions that no one has asked. Why not? Both you and I would.
I was never a missileer (thank goodness), but had plenty of friends who were. Lots of strange things happened on alert, and a few became legend in the process. There was a female missileer on a four-person Titan II crew who engaged in orgies with the three males on the crew. Needless to say, her Air Force career came to a rather abrupt end.
But one of my favorite stories involved a Minuteman III crew commander at Whiteman, back in the 80s. Some of the drills run by SAC were very realistic, and early one morning, the crew commander became convinced that nuclear war was about to break out. So, in violation of all protocol/procedures, he asked the base command post to patch him through to his wife, asleep at their home in Warrensburg, MO. “Honey, it’s World War III, head for the hills,” he shouted into the phone, then hung up and went back to his checklist. As you might imagine, that set off a minor panic among spouses, and within an hour, there was a convoy of frightened wives and children heading for the Ozarks.
That was his last alert....
