What I really would like to know is just how and when did this obscure YouTube video , which had perhaps several thousand views at most, suddenly come to the attention of the Obama White house and to the Department of State in the first place.
Who made them aware of it and for what reason?

Why would Muslims demonstrate before 9-11. They would just go destroy something American on the day of celebration. I seriously doubt there were a bunch of Muslims sitting in an internet café a block away trolling youtube for anti-islam videos completely unaware that it happened to be September 11th. There are thousands of videos to piss them off but they managed to find the ONE video that created by someone that could be thrown in jail to give the appearance that we will punish those who insult islam. A coincidence?

Only the final one was forwarded, by persons unknown, to Sheikh Khalid Abdullah the cleric in Cairo who aired it on TV. Nakoula, the maker of the original film, was not the one who uploaded the final version, Innocence of Muslims, to the internet.

The film was originally entitled Desert Warrior according to excerpts from the call for performers posted on Back Stage and republished by The Blaze.
The Back Stage advertisement for actors noted a character named George, who was a strong leader, a killer with no remorse, a romantic, and a tyrant. The character of Assad would be portrayed as a man with an Israeli accent who was a bearded tribal leader. The character of Hillary was described as a woman who is 18 but looks much younger, features a petite build, and a look of innocence.
