He didn’t look back to see which one was behind him, and somehow the choreography was off. He said they had “switched on me” in the clip. This is about poor stagecraft, not confusion about the identities about the individuals.
There have been lots of posts about this gaffe.
I’m concerned that we’re fixating on little things (which, yes, add up to big) which are unpersuasive to voters at large. Better to attack things like his promise to cure major diseases (as though it simply wasn’t a chosen priority for the last N presidents) and other utopian plans.

This was no big deal. They were both on stage and he got confused as to where each one was standing.
He gets confused all the time and he lies immediately after to make up a reason for it that’s not a good sign either. I watched him last night and his wife and sister always stood on exactly the same side but he says they switched. He knows hes making these mistakes and he tries to cover for it right away.
A lot of things that happen in American politics are unfair. Whether it’s fair or not a lot of Americans have the idea that Biden is losing his marbles. That’s a real effect.
My take also. Pretty much a nothingburger.
Biden is exhibiting widespread frontal release signs and organicity, He is clearly becoming demented, and his family need to be gently, but firmly, guiding him to a simpler existence. Keeping him in the campaign is just cruel
Another unforced error. Joe Biden, front runner or not, can end up sabotaging his own campaign far more effectively than any outside agency.
There used to be a front-line fighter, the P-51 Mustang, toward the end of the Second World War and early in the Korean War, that had to be coaxed off the ground and into the air with a gentle touch, because of the enormous torque of the Allison V-1710 aircraft engine which could dip the one wingtip into the ground, causing a most spectacular “ground loop”, the plane somersaulting wing tip to wing tip down the runway.
Joe Biden has a tendency to hit the throttle just a little too quickly and his mouth is running way ahead of his brain, coming with a similar spectacular “ground loop”.
I saw that also last night. I think this one was an honest mistake - he din't realize where each of them was standing. It certainly doesn't rank up there with last week's declaration that he is running for the United States Senate, being arrested in South Africa, or how "150 million Americans have been murdered by gun violence."
It’s senility and not anything else.
