My first thought is to suggest to go with the S&W 642, which is a .38 Special, but it is hammerless, which is consistent with your frozen thumb.
Perhaps a first step would be to seek one on one training from a qualified instructor, who can address your specific needs.
The instructor can address grip issues which are key to accuracy.
You index limitations are another key concern best served by a qualified instructor.
Shots on target are far more important than any power upgrade.

Makes sense, I’ve used the same gun range since I took my CCW class, same instructor too, 2 ret. Leo runs it. He nixed the hammer-less due to kick back. I’m 30 miles from Memphis, in a small town, so most all shopping eating out, medical is in Memphis. I do most of that alone. DEM run city.

I stumbled across her videos on Youtube a couple of weeks ago and have been watching ever since. There are a ton of them.
Can’t believe such a talent isn’t more famous.
Then its up to you to practice between training sessions.
Also, at home, practice empty gun, draw from conceal and presentation to target drills.
Those mechanics are important to ingrain in muscle memory.
Been looking out on Newsgroups to see if I can find any of her stuff...so far, no luck.
Will definitely check out more of her Youtube postings..kind of an “eclectic” style which can go with many tastes.
She started out with a three piece band, guitar, bass and drums, later expanded the band to include horns, keyboards, fiddle.

How well I know, spent 3 yrs rehabbing the hand. Ligament release has been recommended, I just don’t trust the 2 hand surgeons. If the index finger is the big problem.
I use to quilt now can’t.
