I have put thousands of rounds through Glock 17’s and 21’s. It’s possible to induce a malfunction, but you have to work at it.
You might be able to get a failure to feed malfunction if you use cheap or damaged magazines, which would be my first guess in this case.
There’s a fellow over at a site called the FAL Files who had an FN FAL for years and shot the daylights out of it and never cleaned it. He even named it Ol’ Dirty. Round count is now over 16,000 rounds.

I’ve been shooting Glocks for decades, and I’ve never had one jam on me. Maybe he limp-wristed it; maybe he was using crud ammo; maybe he was using crappy, non-Glock magazines; maybe his pistol was dirty to the max.

This kind of has the feel of a mass shooting similar to school shootings where the main motivation is notoriety and body count. The subway location is novel and may spur copycats
