If you look at one of the pictures of a section that didn’t collapse, it shows ~8-10 parking spaces with another ~8-10 cars parked in the driving isle.
That could easily double the weight on the floor, probably pushing it to the design limit.

IMHO, very likely. Looks like abort 10-12 normal spaces, with 2-3 shoehorned in on the sides and another 9 parked in the drive isles. They appear to have doubled the capacity ion the ONE section of roof.
Who knows what the rest looked liked.

Highly likely. The video from the dash cam of the car in the level second from the top shows how the area that collapsed was the portion of the second deck that was not under cover. The suspicion would be that the area was exposed to leaks that weakened the concrete and steel. The covered areas appeared to hold up.
