I am not sure why transgenderism doesn't fall under the same auspices as body dysmorphia disorder. To me, performing surgery in this situation is akin to doing gastric bypass surgery on someone who has anorexia nervosa rather than providing psychotherapy and medication to treat it as a mental disorder.
