My ob-gyns are similar. You pay up front for non-pregnancy visits. For pregnancy, you pay $100 a month and they submit the rest to the insurance company at the end. You pay the difference. They aren't "in-network" with any company.

There is one receptionist/biller, and one doctor in the office. They don't over-schedule, rarely any waiting. The doctor gets your weight, blood pressure, draws blood - they even do some simple labs in the office themselves. You have plenty of time to talk to the doctor. There are only three doctors so they really get a chance to know you - not just reading your chart.

Now the insurance companies are trying to get a bill passed that they don't have to pay "usual and customary" fees. My doctors tell me their practice will not survive if this goes through.
