John Nance has a wonderful video which you can see part of on his website. He explains how the airline industry changed their culture to improve safety. The medical field needs to do the same. I am glad that my employer is in agreement.

I can’t remember the circumstances/illness, many years ago, but I know the med was to be given once a day to one of my children...
I always read the instructions/dosage of prescriptions before I submit the paperwork to the pharmacist. When I picked up the prescription for my child, I noticed, on the bottle, the printed instructions said to take one capsule every 4 hours.
I did an about face before walking out the door..told the pharmacist about her mistake...
She corrected the instructions on the bottle..never once apologized to me. I realize that apologizing admits guilt, but I expected more.
This is merely another example of the perverted legal system which condones pettifoggers who cast about for the deepest pockets that can be fleeced.
