Of course as far as I know, the only way alcohol (or cigarette) intake is measured is by what the patient tells the doctor. It’s not like they take a blood test. So he likely told the doctor a couple or few drinks a week/a pack a week. Is it true?

Usually the only time a doctor recommends "moderation" of alchohol intake is when a test shows your liver enzymes are out of whack. That means more than 3 or 4 drinks a day for a man. There is a good chance the President has an alchohol problem.
