No, I don't either. But another FReeper - I can't remember which one - said that he did know a Nielsen family when this very subject came up a couple months ago. So far, that person is the only person I have ever say that they even knew a Nielsen family.
But, the advertisers pay incredible amounts of money to access that data, so I'm guessing it's as accurate as it can be, because advertisers want to only spend money where it's effectively reaching their targeted audience. The only "ideology" they're beholden to is the ideology of money.
Because they have incredibly detailed information on the families that actually have Neilsen boxes in their homes - gender, age, height, weight, income, religion, political affiliation - virtually every piece of demographic information available.
Neilsen then tries to mirror the sampling to the demographics across the nation, with each group represented as close to proportionally accurate as possible. There is a tremendous amount of research that goes into it, for the reasons I mentioned in a post just above.
That must have been several years (decades) ago. Neilson now uses an almost entirely automated monitoring system.

Actually, I think in some markets, that information does go into their larger "formula" as does the information they capture from the TiVO boxes.
Several years ago, Neilen reworked how they capture and calculate their ratings because advertisers weren't convinced of their accuracy. While it's not frequently reported, networks and advertisers know EXACTLY what percentage of the larger audience is watching the show on a TiVo box - I'm using "TiVO" to represent not just that particular brand, but all of the DVR that are installed including the propriety cable DVR boxes.
I suspect that many of the viewers were the same ones who gawk at train wrecks.
