According to the picture and caption, we have:
1. Barack’s right hand on the couch next to him.
2. Barack’s left hand on his left leg per the first inset.
3 & 4. Moustache’s right hand and another hand on his right knee, per the second inset.
5. Moustache’s left hand in his lap, underneath his right leg.
That’s 5 hands!
