Secretary Clinton's "Accountability Review Board" (ARB) declared multiple times in its unclassified report that although there were multiple failures in leadership, "management ability," allocation of security resources and communication, the board could not find "reasonable cause" to discipline (or even name) one person in the State Department.
The report states that there was a breakdown in communication between Libya and Washington. It confirmed previous testimony given on Capitol Hill that the personnel in Libya did ask for increased security. However, the ARB found that those working at the embassy in Tripoli "did not demonstrate strong and sustained advocacy" for increased security at the "special mission" in Benghazi. The report goes on to say that the diplomatic security staff in Benghazi in "the months and weeks" leading up to the attack (and on the day of the attack) was "inadequate, despite repeated requests." The ARB found that the security of the Benghazi special mission "was not a high priority for Washington when it came to security-related requests, especially those relating to staffing."
Amazingly, however, the ARB made sure to extensively absolve anyone in the State Department from being accused of being derelict of their duty:

Here are my three questions for Benghazi-Coward B. Hussein Obama:
1.) Are you HIDING INFORMATION from the US Congress about the Benghazi Massacre?
2.) Have you been LYING to the US Congress, The Media, or the American Public about the Benghazi Massacre?
3.) Were you, as Commander in Chief, GROSSLY NEGLIGENT in your responsibilities to protect and assist those four Americans who subsequently were murdered in the Benghazi Massacre?

So...if we’re gonna conduct our overseas affairs so chaotically and destructively,...why have any? why have a Federal Dept of State? Which of the 50 states so desperately need Federal State Dept representation in Libya that we have to sacrifice fellow Americans on the altar of ineptitude Her Heinousness Hillary and her Boy Barack have erected?
What if instead the WH came under attack?
I WONDER if there would be no response for 8 hours...? Maybe just four guys would shoot back, and gradually get killed..?
Even after everything that has happened, this is almost unbelievable.
Who in their right mind would want to work for this administration? An ambassador and 3 others are dead and nothing is done about it.
Double speak...the truth is that any request for heightened security was for the Embassy in Tripoli, the whole country was going unstable...and the truth lies in the fact that those of the upper eschelon did not regard more security for the ‘special mission’ in Benghazi in high priority because they knew it was CIA staff running guns...The lie is who the ‘upper eschelon’ was, since Hillary Clinton did not have any authority over it whatsoever. It was the White House that did not make providing security for Benghazi priority because those CIA operatives were loaded in guns and in fact were dealing in them with terrorists. They were caught off guard and there wasn’t enough of them shooting back. Also, they must’ve just shipped all the mortar weapons out. In fact, if it hadn’t been for Tyrone Wood and Glen Doherty breaking rank and rescuing the ones at the ‘secret mission’ house, they would all 37 of them be dead. There hasn’t been a ‘consolate’ in Benghazi since Obama allowed al Qaeda, the Muslim Brotherhood ‘civilian army,’ to take over the country and run it. The ‘report’ is nothing but one big lie in and of itself.
