Experimental drug likely saved Ebola patients
CNN ^ | 08/04/2014 | By Dr. Sanjay Gupta and Danielle Dellorto

Three vials containing an experimental drug stored at subzero temperatures were flown into Liberia last week in a last-ditch effort to save two American missionary workers who had contracted Ebola, according to a source familiar with details of the treatment.

The drug appears to have worked, sources say. Dr. Kent Brantly's and Nancy Writebol's conditions significantly improved after receiving the medication, sources say. Brantly was able to walk into Emory University Hospital in Atlanta after being evacuated to the United States last week, and Writebol is expected to arrive in Atlanta on Tuesday.

On July 22, Brantly woke up feeling feverish. Fearing the worst, Brantly immediately isolated himself. Writebol's symptoms started three days later. A rapid field blood test confirmed the infection in both of them after they had become ill with fever, vomiting and diarrhea.

It's believed both Brantly and Writebol, who worked with the aid organization Samaritan's Purse, contracted Ebola from another health care worker at their hospital in Liberia, although the official Centers for Disease Control and Prevention case investigation has yet to be released.

A representative from the National Institutes of Health contacted Samaritan's Purse in Liberia and offered the experimental treatment, known as ZMapp, for the two patients, according to the source.

Brantly received a transfusion of the blood of a 14-year-old Ebola survivor who he personally helped to treat. Giving blood transfusions from survivors to still suffering Ebola patients is an established, though not nearly proven, treatment for the largely untreatable disease.

However, on Thursday charity SIM said in a statement that Mrs Writebol’s condition had worsened, despite the serum.

Her husband, David, is close by but can only visit his wife through a window or dressed in a haz-mat suit.

‘We continue to pray for Nancy’s full and complete recovery,’ said Bruce Johnson, president of SIM USA. ‘Even though her condition has worsened, we know she is receiving the best possible medical care, and we are thankful that she has access to this experimental drug.

Interestingly, I read that there was only one dose of this, and the doctor (Brantley) gave it to the volunteer (Nancy Writebol).

I had read that doctors a few years ago (maybe in Nigeria?) had tried the blood transfusion system and they thought that it had worked. It’s basically the same as any treatment for a disease like this, based on antibodies in the blood of a survivor, but without the intermediate culturing and refining phase.

However, if it’s true that the doctor did not receive the vaccine is doing better while the patient who is doing worse is the one did receive the vaccine, I think the article was not exactly on target.

Furthermore, the doctor was younger, and the two were opposite sexes, so it would be necessary to see what happens with two comparable subjects (i.e., both white males in their 30s or 40s, or both white females in their 50s or 60s).

However, on THURSDAY charity SIM said in a statement that Mrs Writebol’s condition had worsened, despite the serum.

*********************************************

Actually, Writebol improved after given a second vial of serum (the report she had worsened was Thursday, the improvement was Sunday):

Writebol also received a vial of the medication. Her response was not as remarkable, according to sources familiar with the treatment. However, doctors on SUNDAY administered Writebol a second dose of the medication, which resulted in significant improvement.

She was stable enough to be evacuated back to the United States and is expected to arrive before noon Tuesday.

Oh God please let it be true; if so, this means that this disease is now both treatable and curable.

This also means that the people who created this are going to get the Nobel Prize and the gratitude of thousands.

If both infected specialists are able to recover, I am certain this will make headlines around the world for months.

That is what I was lead to think as well. Now it sounds like there was 3 vials? One for Brantly and 2 used on Writebol?

Thanks for filling in the blanks...

Although I thought there wasn’t enough serum for both of them to begin with, let alone a second dose?

At least that was what earlier reports stated. Brantly gave her the only dose...?

The articles I have read stated that there was a total of three vials brought overseas, but they had to thaw naturally. Dr. Brantly was to be given the first vial (after it thawed), but he offered it to the other victim. But once he took a turn for the worse, it was given to him instead. the other patient was given the second vial (and then the third), after they were fully thawed.

I question the accuracy of the of the original article, I have not read anywhere else regarding this: ... Brantly received a transfusion of the blood of a 14-year-old Ebola survivor who he personally helped to treat. Giving blood transfusions from survivors to still suffering Ebola patients is an established, though not nearly proven, treatment for the largely untreatable disease.

Has anyone else seen this reported anywhere other than this article?

Although I thought there wasn’t enough serum for both of them to begin with, let alone a second dose? At least that was what earlier reports stated. Brantly gave her the only dose...?

********************************************

Here's the part of this article that clarifies the his/her dose situation (at least 3 doses on hand, issue was thawing) - the first article on this, where he gave her "his" dose, was written in an inaccurate manner, IMO & led you to believe there was only 1 dose between the two of them.

The ZMapp vials reached the hospital in Liberia where Brantly and Writebol were being treated Thursday morning. Doctors were instructed to allow the serum to thaw naturally without any additional heat. It was expected that it would be eight to 10 hours before the medicine could be given, according to a source familiar with the process.

Brantly asked that Writebol be given the first dose because he was younger and he thought he had a better chance of fighting it, and she agreed. However, as the first vial was still thawing, Brantly's condition took a sudden turn for the worse.

Brantly began to deteriorate and developed labored breathing. He told his doctors he thought he was dying, according to a source with firsthand knowledge of the situation.

Knowing his dose was still frozen, Brantly asked if he could have Writebol's now-thawed medication. It was brought to his room and administered through an IV. Within an hour of receiving the medication, Brantly's condition dramatically improved. He began breathing easier; the rash over his trunk faded away. One of his doctors described the events as "miraculous."

By the next morning, Brantly was able to take a shower on his own before getting on a specially designed Gulfstream air ambulance jet to be evacuated to the United States.

Writebol also received a vial of the medication. Her response was not as remarkable, according to sources familiar with the treatment. However, doctors on Sunday administered Writebol a second dose of the medication, which resulted in significant improvement.

On July 22, Brantly woke up feeling feverish. Fearing the worst, Brantly immediately isolated himself.

That was a Tuesday. Then there's this from his wife's sister in law dated Sunday, July 27, "Amber and their kids arrived back in Texas last week" - http://www.carrollcrossroads.com/ . I still think he shipped his family home on the next plane out when he first suspected/knew he had contracted it.
