Why is mental condition in quotes?

I have reviewed literally thousands of medical records and never yet found one reporting "mental condition." Neurological, psychiatric, yes. never "mental." This is the equivalent of a note from your mommy.

Why is mental condition in quotes?

It means that what is in the quotes is a euphemism.

For instance, 'excellent' is a euphemism for 'possessed by demons'.
