I really don’t think this will make much difference. As opposed to auto insurance where you are largely looking for someone to just cut you a check when you have an accident, health insurance is more about buying into a network of doctors and hospitals that can use. Can an insurer operating out of Nevada set up a network in Alabama that would be more attractive to consumers than an insurer operating in the state?

One caveat here. I am in favor of voluntary interstate compacts as long as the Federal government will not have any regulatory oversight. Typically, when things cross state lines, the Feds get involved with their mandates and regs and they essentially turn it into a federally regulated program and you are back to something like 0-care
If there are disputes among the states with respect to insurance compacts, they can be resolved on a case by case basis in the Federal courts as was originally intended for interstate commerce. However, we have to realize that the current annotated constitutional interpretation of the commerce clause means that the Feds take over when things cross state lines.
