Virginia is facing a huge bill for unexpected Medicaid costs that hamper proposed new spending on things like school improvements or tax breaks for the poor. State officials said Friday that Virginia has about $460 million in unforeseen Medicaid costs. Wells Fargo & Co. acknowledged Tuesday that it failed to offer mortgage modifications to hundreds more borrowers than were due them and that many ultimately lost their homes because of the error. *snip* Now, in a regulatory filing released Tuesday, the bank said it improperly denied modifications to 870 borrowers — nearly 40% more than first thought — and that 545 of those borrowers later lost their homes.

The confirmed cases became ill between September 26 and November 5, according to the health department. The number has risen from 18 cases, including six deaths, announced last month by the health department. A staff member was also affected by the outbreak but has recovered.
