"Over the past few years, I’ve been keeping track of Russia’s annual launch failures. For reasons I can’t quite recall, the table I’ve used only went back to 2009. Recently, I saw a graphic on a Russian website about launch failures, and I realized I hadn’t gone back far enough. So, I dug into the records of the last 30 years from 1988 through 2017, which covers Russia and the last four years of the Soviet Union. And holy crap! There were a helluva lot of them. Launch failures are not a bug in the system, they’re a feature..."

Had an odd thought. If Gregarin had taken Komarov’s place as he was alleged to have tried, would Komarov been flying Gregarin’s MiG-15 the following year?

The US had plenty of vehicle failures too, especially in the early days. Two resulted in fatalities (Challenger,1983 and Columbia in 2003. Apollo 1 was a ground test but it should count too. That’s 17 crew lost. Apollo 13 was a really close call.

Lots of failures and flaws that should have deadlined the vehicles and missions until they were fixed were ignored due to time, political and monetary pressures.

Sounds right. Loral acquired a Honeywell division, and was frequently mentioned as the division involved in helping the Chicoms fix their problem, and giving them the knowledge to more accurately target the ICBMS pointed at the US.

Both space shuttle failures were caused by the ozone hole.

They changed the type of outside insulation so that it contained no CFC's or HFC's. The new insulation proceeded to fall off in big chunks, at the wrong time.

So in reality, the ozone hole didn’t cause the failure,
excessive environmental concern caused the failures.
