“Interesting article. People still dont know what NPs or PAs do. 30 years since the onset of NPs as a profession they still see us as nurses. I have often advocated for the change in the title. I think it misrepresents us as still being nurses. Nurses follow doctors orders. They are task oriented. Depending on their specialty they can observe, document, assess, suggest, but they cannot diagnose, prescribe, nor run a code.
The profession started as a means to do basic medical functions such as physical exams and take care of the colds and runny noses. If a condition required more complex analysis then we referred to physicians. We dont take anything away from physicians.
The fact is that young medical students are not going into general practice, family practice, or internal medicine after medical school. Theres not enough money in it for them. The universities keep pushing up the cost of a medical school education. The school loan companies are completely unforgiving in their handling of student loans. They are loan sharks in every sense of the word and it is BIG BUSINESS. Why should you be in hock for $200,000 of premed and another $300,000 for medical school and then another unheard of amount for family practice?
Most family practice physicians stop delivering babies because the liability insurance is through the roof. You get mothers who get late or nonexistent prenatal care. They smoke. They drink. They do drugs. Then when they have complications or the baby is born with problems they blame the doctor and sue them.
Its easy to point fingers when it comes to health care. Its easy to create generalizations and make believe scenarios. Health care is one of the most complicated issues on the planet right now. As if we dont have enough of them. Try reading Obamas health care proposal for a mouth opening, dry and convoluted document. I still cannot believe he had people with actual university degrees helping him on that piece of rotten crap.
Anyone who has ever taken a medical ethics class as I have know that when it comes to health care sometimes there are no right and wrong answers. We as Americans have a lot to be thankful for. We have, for the most part, dedicated people who do go into the health professions, from doctors to medical assistants mostly because they want to help people. Some do it because its a good job. Try getting your health care in Calcutta as a comparison. There is none.
But we are spoiled. We want to eat as much as we want, exercise as little as we want, smoke anything we want, drive our cars to the corner store and consume, consume, consume. And we dont want to be accountable for anything.
The television peddles a magic pill for everything from gas to erectile disorder, and we buy it all. Our health care system is up to the gills with sick people. People want to continue to smoke and eat and gain weight without any accountability, and its always someone elses fault.
I am currently over-paneled by almost 200%. That means I am seeing over 100% of my patient limit. It takes 3 months to see me. If my profession was obliterated tomorrow it would take over a year for any of my patients to see the podiatrists, orthopedics, plastic surgeons, oncologists and otolaryngologist specialties.
I studied hard for my degree and I learned from some of the best doctors, physician assistants, and nurse practitioners in the industry. I cannot speak to the issue of foreign doctors, but I have worked with some quite good ones and some questionable ones. Same with the American educated.
I do not say I have any grand solutions, but I do say there are a great deal of misconceptions of what health care means today in the 21st century. And I always find it amusing to hear people say they want a real doctor without having the slightest idea what that is and how it applies to them personally. What I can say is that the times I have diagnosed something like an extremely rare form of dwarfism or caught a femoral aneurysm the patient and the family were very grateful. “

Veterans Affairs workers reap bonuses despite claims backlog

Staffers in the Department of Veterans Affairs have been reaping millions of dollars in bonuses despite a huge backlog of cases that has veterans waiting hundreds of days for compensation.
The bonuses were reported Monday by journalism project News21, whose findings were published in The Washington Post.
The report said that in 2011, more than two-thirds of claims processors received a total of $5.5 million in bonuses. That same year, the backlog of claims swelled by 155 percent.
The bonuses reportedly created a perverse system, where workers were encouraged to stick to simple cases in order to meet certain performance benchmarks — in turn ignoring claims that required additional attention.
“At the beginning of the month ... I’d try to work my really easy stuff so I could get my numbers up,” Renee Cotter, a union steward for a Reno, Nev., chapter, reportedly said.
