You need their date of birth and their SSN (if they are in the US).

Wrong opener. "Insha' allah" means God willing but is normally used when speaking of something in the future.
Salaam alaykum (Peace upon you) is the most common. Or 'bism Allah Alrahman Alrahim' (in the name of God the most merciful).
It has been awhile since I've written Arabic down.
