Way back in the day I worked on barge vertical market software. Accidents happen all the time, reporting was built into the software. Matter of fact, our big edge at the time was that we were one of the first to hook into GPS so as to do constant position recording (for insurance, etc.)

Used to live in the Mid Ohio River Valley. Loose tow barges striking bridges and other infrastructure was not an uncommon event. Never catastrophic damage, but inconvenience as bridges are closed temporarily for inspection.
