The document in question just reads "Mombassa", neither signifying Kenya or Zanzibar. From reading here, it appears that Colonial Kenya considered Mombassa as part of Kenya due to Crown agreements earlier in the century. I do not find this as a deal breaker.
The issue of Coastal Provincial General Hospital vs Coastal General Hospital is also not necessarily a deal breaker. Either sloppiness, or local colloquialism could shorten the name.

The only way to ascertain the truth is to research the numbers listed on the document. If it shows that there is a link, then imho, the document is real.
