Fox had an expert on that described a possible way it could happen, if it startled the engineer, who might have reflexively jerked, and lost control of the speed.
But, I don’t know enough about train to know if his explanation made sense.

Yes, if it went through the windshield, something hit the conductor, and knocked him senseless.
This sounds implausible, but this guy doesn’t strike me as someone who didn’t take his job seriously. He always wanted to be a train conductor (I still don’t know what I want to do when I grow up). Another possibility (again, implausible, but possible) is undiagnosed epilepsy.

What if the projectile was a diversion for a hacker to take control of the train? Why would it be any different from the hacker and the plane?
