I am a patient with this medical group. MKMG is like the Borg around here.
While it is possible these types of records would be printed, this group does everything electronically. Ask yourself, "why would someone in the group print this"?

Until confirmed, stay away from this. But use the “short-circuited” comment often. It suggests a certain detachment/disconnect from reality.

Patient records? I see them as an independent diagnosis. For example, I could say, “Reno89519 is a raving lunatic,” but without credibility it means nothing. No offense meant to Reno89519, of course. ;-)

Primary Care group I was using were crapping in their pants about the unknowns of Obamacare. Talked of closing their practice and retiring. Within 6 months after passage they merged with another group, moved to a bigger location and invested in new diagnostic equipment.... they found a golden nugget somewhere in those thousands of pages.
May as well have the original birth record for bammy in Kenya. This will be totally ignored by the media.
If this is fake then this doctor has a lawsuit, and would definitely be out there refuting this.
The “leaked letter” could very well be bait that Team Clinton is hoping that Trump will run with.
