They are ballots required by the Fed gov that must be given out if the person who came to vote wasn't on the list but challenged and said they were registered. They get a provisional ballot and vote it and then that ballot gets checked later to see if the person was registered and entitled to vote, if they were the ballot counts.
And you expect a handful of this kind of ballot error in any state, maybe a possible few hundred at worst. If Ohio has 150,000 something is rotten in Dayton, or somewhere.
