Funny - I had the same experience. First the dental hygenist scolded me for not flossing correctly and then got offended when I told her “That’s what I pay you to do”.
Then the clerk at the front desk pulled the same deal about updating records. She wouldn’t accept “Same as last before” so I took the form and filled it out....”Same as last before”.
She summoned the dentist and I told her almost the same verbatim. In her case however she stopped dead in her tracks and looked me square in the eyes and then said, “Fair enough”.
Good deal because she’s a great dentist ;’)

If all the med records are available to all health care providers why should we have to fill anything out. It is all in our records that are waiting to be highjacked and snooped in for nefarious reasons.
My Dr said that he received something addressed to Dear Healthcare Provider rather than Dear Dr. _______. They are trying to demean the Dr’s and advancer the PA to level the playing ground and reduce the payments.
