I have encountered this in the past. Where a dominant attached spirit created an identity in the host of their own gender.

Problem is, if this is not resolved at an early age, the hosts individual identity does not develop and they go into a severe depression when you remove the attached spirit.

Tired&retired, spirits do not have a sex, since sexual differentiation is a biological reproductive characteristic and spirits do not reproduce and are not biological: they are disembodied.

Any and all gender characteristics assumed by a disembodied spirit would be artifice and/or delusion.
