How can someone tell a baby isn’t theirs just by looking at it?

I used to work with a woman who was married to “Max” and having a not-too-well-concealed affair with “Joe” that most of us at work knew about. She got pregnant and the baby was born looking exactly like “Joe” even though she played it off like “Max” was the father. Some people have certain features (a prominent nose or eye shape or whatever) so it doesn’t necessarily have to be a race thing, though that thought did cross my mind, too.
