Sugar and grains are the real problem.

Oxidized LDL particles penetrating the arterial wall, inciting inflammation and damaging the arterial tissue causes atherosclerosis. It is not caused by fat magically attaching itself to the surface of the arteries like fat in a kitchen pipe.

So eat the burger but skip the bun.

I was going to say “Hold the bun” but you beat me to it. I would skip the tomatoes too because carbohydrates.
