Broken picture icon. It gives a 403 Forbidden response, because the Referer header is set to FR.
If you copy the image location and paste it into another window, it shows up fine (because you requested it without sending a Referer). If you then go back and refresh the FR page, the image will now show up because your browser retrieves it from the cache instead of requesting it anew. That is also why it works in the Preview before posting.

It's unquestionably because it's a hotlink. Planetperplex wants to forbid hotlinking, either from FR or from everywhere. Given the nature of the site, I suspect it's the latter. Some sites will go even further, requiring that there be a Referer and that it refer to the site in question. To defeat those, you need a browser extension to spoof the Referer.

The FR server isn't involved at all, other than to serve up your HTML. It never handles the image. The HTML goes from FR to your browser. Your browser then parses it and tries to gather in all the external resources it refers to, such as this image. That process takes time and bandwidth, so your browser will cache previously loaded resources for a limited period of time.
The reason it's so confusing is that, typically you will find an image, for example on planetperplex.com and copy the URL into a post. It appears to work for you because the image was entered into the cache when you browsed for it on planetperplex. Therefore, your browser doesn't need to request it from planetperplex again (which would fail). But everybody else gets the 403 Forbidden response.
