25th Amendment
Section 1. In case of the removal of the President from office or of his death or
resignation, the Vice President shall become President.
Section 2. Whenever there is a vacancy in the office of the Vice President, the President
shall nominate a Vice President who shall take office upon confirmation by a majority
vote of both Houses of Congress.

I have always believed the 3rd person was the Speaker of the House.

That would be true if both the President and VP were gone. But if the VP is elevated
then the 25th becomes in play and a new VP is appointed subject to approval by both
the House and Senate.
Maybe it used to be that if the President and the VP were both killed, or died .. the Speaker of the House would assume the head of the government.
Maybe that’s what I was thinking ..??

You are correct and that would still apply today. The 25th was ratified in the 1960s I believe.
It provided for a new VP to be named and approved by the House and Senate if the President
was replaced by the VP. Before then no procedure was in placed to provide for a new VP
should he be elevated.
