Ever try to "Have it your way" at any McDonalds?
Their eyes glazed over, all production stops, panic ensues.

When IO was a teenager, when time wasn't as precious, all of the time. I order a Quarter-pounder with cheese and "extra everything". It ensured I got a fresh-cooked burger every time.

Back in the day, all were told you cannot make changes to the product because of trademarks, this was the actual corporate policy. As the years went by so did that policy and since all the food is now pre cooked and held as separate components you can pretty well get anything you want, if you pay for it. As for me, after 22+ years with McD I don't care for the taste since they stopped cooking, assembling and selling right off the grill and that goes for BK, J in Box, Carls, et al. If I want a good tasty burger I go to a small mom and pop or In N Out as long as I get to eat it right away.
